SLOAN COMFORT CARE HOME AT A GLANCE
Sloan Comfort Care Home was built through a partnership between the Accountable Health Community, Inc., a not-for-profit organization formed to address disparities in healthcare, and St. Andrew's Church in Sloan, NY. The Home offers an alternative for hospitalized patients, who do not wish to die in an institution (hospital or nursing home) or for people in the community who for a multitude of reasons cannot receive end-of-life care in their own home.
